Will I have the ability to continue benefits coverage?
Yes. If you are enrolled in Company medical, dental, vision or Healthcare FSA benefit plans, you have the right to continue benefits coverage pursuant to COBRA. You will receive information on COBRA at your home via regular mail within 14 days of your Termination Date with instructions on how to sign up for COBRA and the cost of each coverage option. You will also be able to enroll on-line for your COBRA coverage at www.Benefits.EssilorLuxottica.com. Please allow up to 7 days from your Termination Date for the on-line system to be updated.

Will there be a gap in my coverage?
If you enroll in COBRA within 60 days of your Termination date, you will not have a gap in your benefits coverage. Please refer to the COBRA packet for specific information.

What if I have a balance in my HSA Account?
If you have a balance in your HSA Account, your account will remain active and can be used for eligible health care expenses (medical, dental, vision). You will be responsible for the monthly account management fees.

What happens to the balance in my Health Care FSA Account?
You can claim up to the full goal amount in your HC FSA as long as the claims were incurred while you were an active employee or participating in the FSA on COBRA. You have until March 31st of the following year to submit your expenses.

How much will COBRA cost?
A summary of COBRA costs, as well as details on invoicing and payment due dates, will be included in the COBRA materials sent to your home following your Termination Date. If you would like COBRA information prior to receiving your packet at home, please call 866-431-8484, Options 1, 3 and 1. By selecting these options, your call will be directed to Alight. A Customer Service Associate will be able to provide you with the necessary information. Please have your Login ID and password available for verification purposes.
Does our life insurance plan provide for the option to purchase ongoing life insurance benefits after my employment ends?
Yes. Certain life insurance policies can be converted by contacting Anthem Life at 1-800-801-6142 within 30 days following your Termination Date. The actual form that you will need to submit will be included in your COBRA packet that you will be sent in the mail to your home following your Termination Date.
If I am a member of the Credit Union, can I continue my membership?
Yes. Members of the Credit Union may continue to participate in the Credit Union indefinitely by maintaining a minimum balance of $5.
I have not used my annual Complimentary Eyewear Certificate yet. Can I still use it?
You may use your Complimentary Eyewear Certificate up until your Termination Date.
I still have some Friends of Luxottica Certificates. Can I still use them?
Yes, Friends of Luxottica Certificates can be utilized until their expiration date.
